1. Title the email with your name and the location of your chapter.
2. Tell us a little bit about who you are and the reason why you want to start this chapter. We want to get to know you!
3. Provide us information about your chapter which must include the program name, area/location, target audience you intend to impact, proposed start date, along with whatever else you feel the need to include.
4. Explain what we can provide for you beyond the platform and guidance.
5. Provide a short mission statement.
6. Provide a list of your officer titles (or proposed officer titles given your application was accepted).
